A separate VIDS/MAF work request is required for like items that have different manufacturer's code/ part numbers. In cases of egress and survival equipment with like part numbers but different manufacturer's codes, enter five zeroes in the manufacturer's code block and the time/cycle block using the appropriate prefix.
DISCREPANCY BLOCK, Enter descriptive narration, serial numbers if appropriate, MRC numbers if applicable, and signature of the maintenance control supervisor.
The VIDS/MAF work request is delivered with the component and/or items to the IMA production control. The IMA production control supervisor signs the VIDS/MAF work request in the corrective action block and returns a signed copy 2 (as proof of turn-in) to the requesting activity. Copy 2 is placed on the maintenance control/phase VIDS board until IMA has completed the check, test, or service action.
When the item(s) that have been tested, inspected, or serviced have been reinstalled, the documentation of the VIDS/MAF that has been held in suspense is completed. Refer to figure 4-35 and locate each block as you read about it in this section.

The work request, customer services form (fig, 4-36) is used by IMA and supply department personnel to request customer services support from depot-level activities. Customer services support is limited to services not involving repairs within the capability of the requesting IMA.
